How do you care for my ongoing needs?

Our approach to care management and the Care Connect service is a particularly valuable program. You may benefit from our care management program if you have at least one diagnosed chronic illness or disease. Our services allow us to stay on top of any changes in your health. This level of accessibility and responsiveness helps to keep you healthy, happy, and out of the hospital.

How does Care Connect fit into my care management?

Care Connect allows you to receive health care services from the comfort of your home. It uses telehealth capabilities to aid in managing hypertension.

You will be provided with:

  • A cellular blood pressure cuff, which allows one of our dedicated providers to monitor your blood pressure remotely
  • A nurse/care coordinator who largely monitors your health and provides support when you can’t be at the office
  • An optional mobile application, which helps you to stay on top of and respond to any changes in your vitals

How is my health monitored?

You will be assigned a dedicated nurse/coordinator who takes the mystery out of getting and keeping your blood pressure in check. Depending on what they find and see when monitoring your numbers from our office, you may receive medical, lifestyle, and nutritional recommendations. Your coordinator can also help to set up and manage any visits to our office and to other specialists’ or providers’ practices, as some of the more immediate actions that may be necessary could include in-office health care services.

So you provide medication management, too?

Certainly! We understand that medications may be an important part of your heart-healthy program. It is important for us to track any drugs or supplements that you may be taking. By keeping pace with this aspect of your health journey, we can also manage any potential side effects and avoid contraindications.

Dr. Randall Huling and Dr. Edward Eldred lead Olive Branch Family Medical Center with a shared commitment to compassionate, patient-focused care for families across DeSoto County.

Dr. Huling, a board-certified family physician and Fellow of the American Academy of Family Physicians, founded OBFMC in 2003 after serving as Clinic Director, Chief of Primary Care, and Residency Director at Womack Army Hospital during his U.S. Army service. He continues to teach as a Clinical Instructor at the University of Mississippi Medical Center and the University of Tennessee

Dr. Eldred, also board-certified in Family Medicine, joined OBFMC in 2010 after completing his residency at the University of Mississippi Medical Center. A lifelong DeSoto County resident, he earned his medical degree from Kansas City University of Medicine and Biosciences and his MBA from Rockhurst University.

Together, Dr. Huling and Dr. Eldred are dedicated to delivering trusted, evidence-based care that promotes lifelong health and well-being.
