When enrolled in our Care Management Program, you have a dedicated nurse who is there to give you a more intimate, personal, and interactive experience to expedite assistance for all your health needs.
Dedicated, Licensed Care Coordinator
For patients with one or more chronic illnesses
Your coordinator will be your point person for all your health care needs at Olive Branch Family Medical Center — he/she will work with you to monitor your health in between office visits, set goals to help manage your chronic conditions and set up appointments with specialists when needed.
Your care coordinator will be available to you via phone Monday – Friday from 8:00 am to 4:30 pm as well as by email or telehealth.
This program is part of your Medicare Part B benefits and may incur a co-payment depending on your insurance coverage.
